(For exception to this policy, please see Student-Athlete Try out Policy.)

Exit Physical
All student-athletes will be required to fill out an exit physical form at the end of their competitive season. This form will ask the student-athlete to list any medical problems that they have. Based on information given, follow up referral will be made to the appropriate physicians. This is done in an effort to provide appropriate medical care for student-athletes after the competitive season and prepare any returning student-athletes for upcoming seasons.
